Thomas Neill Cream, a doctor of considerable skill and charm, establishes what he advertises as an unconventional abortion clinic in the heart of the city, his silver tongue and confident bedside manner assuring anxious young women that they have nothing to fear in his capable hands. Yet behind the closed doors of his practice, darkness stirs-a sickening compulsion that drives him to perform horrifying procedures that leave his patients dead rather than healed, their lives extinguished while no one dares suspect the refined gentleman who attends them with such apparent concern. When the whispers begin to grow too loud and suspicion starts to stir in the shadows, Cream flees across the Atlantic to Chicago, where he continues his dreadful work with the same calculated cruelty, preying upon vulnerable women who trust their healer.

Eventually captured and brought before authorities in America, he makes a chilling admission to his crimes, his cold detachment never wavering even as the judge's gavel condemns him to spend his remaining days within the stone walls of Joliet prison in Illinois.

Yet justice proves elusive for the monster behind the mask, for Cream's wealthy family leverages their considerable connections to secure his release, freeing a tiger to roam once more. In 1891, the Lambeth district of London becomes his hunting ground once more, and the foggy alleys witness a renewed reign of terror as Cream indiscriminately poisons several more women, leaving macabre notes that frame innocent men for his handiwork, delighting in watching suspected innocents suffer for crimes they did not commit.

His twisted game reaches its inevitable conclusion when authorities finally catch up with him for the final time, and in 1892, Thomas Neill Cream stands in the notorious Newgate Prison, the hangman's rope awaiting his neck. Just before the noose tightens around his throat and eternity claims him, he lets out a final, terrible cry to the assembled crowd below, a confession that would echo through history's darkest corridors-his final, boastful claim that he is Jack the Ripper, the monster who had terrorized Whitechapel three years earlier, though historians continue to debate whether this was truth, madness, or simply one last twisted manipulation from a man who had spent his life deceiving everyone who crossed his path.
